Purchase insurance for student devices.  This is recommended for all students in 6th grade through 12th grade.  If you do not purchase this insurance, you are liable for the full price of the device in the event of damage or loss.  PLEASE USE STUDENT NAME AS GUEST NAME.  IF YOU ARE PURCHASING FOR MULTIPLE STUDENTS, PLEASE LIST ALL STUDENT NAMES IN THE GUEST NAME BOX.
